If you want to start an 'ethical' business - in which you first carefully consider the backstory, habits, and personality of every potential customer to ensure they're fit to purchase (rejecting any you deem not quite responsible or intelligent enough), by all means go ahead. All the best.

No idea what healthcare has to do with this. But FTR, I consider public healthcare a fundamental to equality. When paired with public education, it levels the playing field entirely. With that as your base, it's very easy to see who's genuinely needy, and who is simply exploiting the system.

Private business, control cost to operate the most efficiently and get a raise and a promotion.

Government, protect the budgeted amount, spend it all and ask for more the next budget and get a raise and a promotion.

Private business, either operate efficiently and within the budget allowed or go out of business.
